OC08.06: Is neurosonography predictive of <scp>MRI</scp> findings after co‐twin demise in monochorionic pregnancies?

Abstract
Demise of one fetus in a monochorionic (MC) pregnancy is associated with a significant risk of hypotensive ischemic injury in the surviving twin. Our aim was to evaluate whether neurosonography was predictive of magnetic resonance (MR) findings following co-twin demise. We reviewed all MC pregnancies with co-twin demise who were evaluated with neurosonography and brain MRI (diffusion-weighted and T2) at our tertiary care center. Pregnancy demographics, ultrasound and MRI findings and obstetric outcomes were collected between 2005 and 2015. 68 MC pregnancies with co-twin demise, including 61 MC/Diamniotic (DA) twins, 4 Monoamniotic (MA) and 3 multiples including an MC/DA pair were reviewed. Of these, 31 had Twin–twin transfusion syndrome (TTTS), 16 of which demised before treatment and 15 which underwent laser ablation of placental anastomoses, 19 had selective intrauterine growth restriction (sIUGR), 3 were MA twins, 13 had an unexplained intrauterine fetal death (IUFD) and we could not evaluate the reason for IUFD in 2 cases. Of all normal fetal brain ultrasound (including structural findings and/or MCA values), 71% (24/34) had normal MRI evaluation and 29% (10/34) had abnormal MRI findings (p = 0.005). Of all normal middle cerebral artery peak systolic velocity (MCA PSV), 66% (25/38) had normal MRI evaluation and 34% (13/38) had abnormal MRI findings (p = 0.02). Brain ultrasound evaluation and MCA PSV alone, was predictive of abnormal MRI findings following co-twin demise in MC pregnancies. The addition of MRI evaluation, especially diffusion weighted MRI, found additional features in up to 29%.
