Der intraoperative Einsatz von rahmenlosen stereotaktischen Navigationssystemen ermöglicht die exakte Visualisierung der Sonden- oder Instrumentenposition im Patienten in präoperativen CT/MR Datensätzen in Echtzeit. Die meisten verfügbaren Systeme erfordern eine zuverlässige Fixation des Patienten am Operationstisch. Die Fixierung mit Klebeband hat sich für die Kopf-Hals-Chirurgie als unzureichend herausgestellt, da die Elastizität des Bandes und die von ihm induzierte Weichteilverschiebung der Haut keine zuverlässige Patienten-Bilder-Registration ermöglichen. Die von uns entwickelte Kopfhalterung realisiert eine nichtinvasive Immobilisation des Kopfes über einen an einem Carbongestänge montierten individuellen Oberkieferabdruck, der durch Anlegen von Unterdruck exakt positioniert wird und der zudem Referenzpunkte für die Patientenregistration definiert. Die Modularität des Halters erlaubt eine exakte Repositionierung des Patienten in der Lage, in der die Bildgebung (CT, MR) durchgeführt wurde. Somit eröffnet sich eine Vielzahl von Einsätzen ausserhalb der «klassischen» HNO-Chirurgie, wie etwa Brachytherapie. Die Vogele-Bale-Hohner-Kopf-halterung wurde bis jetzt bei 25 Operationen in den Nasennebenhöhlen und Eingriffen an der vorderen Schädelbasis mit dem Viewing Wand System (ISG Tech., Canada) erfolgreich eingesetzt.
