En paises como Canada, Estados Unidos, Reino Unido y Japon se estan utilizando el Modelo de Manejo de Casos como estrategia para lograr la garantia de la calidad de la atencion y la contencion de costos. En Mexico, la practica de enfermeria tradicional, propicia la fragmentacion de la atencion, lo que ocasiona resultados negativos como complicaciones y mayor numero de hospitalizaciones. En esta investigacion se aplica el Modelo de Manejo de Casos en pacientes con Diabetes Mellitus tipo 2 en un centro de salud comunitario para valorar su impacto en el control glucemico de los pacientes a traves de la hemoglina glucosilada (HbA1c). El estudio fue de tipo pre-experimental, la poblacion fue 21 pacientes adultos con diagnostico de diabetes mellitus tipo 2 y el muestreo fue no probabilistica a los cuales se le valoro la hemoglobina glucosilada y colesterol, asi como la valoracion de sus conocimientos respecto a la enfermedad y tratamiento; la intervencion fue de 12 semanas. La implementacion del modelo en el area comunitaria permitio verificar que este es un metodo efectivo para el cuidado de pacientes con diabetes mellitus tipo 2. Los pacientes participantes en el estudio mostraron disminucion significativa en las cifras de HbA1c, glucemia capilar e IMC asi como en las cifras de colesterol, aunque esta ultima no fue significativa. Al final del estudio los pacientes mostraron mayor conocimiento respecto a la enfermedad, su tratamiento y que hacer cuando se incrementan o disminuyen los niveles de glucosa en sangre y la tecnica correcta para el cuidado de los pies. Respecto a las actividades de autocuidado mejoraron el apego al tratamiento, al regimen dietetico y al ejercicio. En cuanto al tratamiento con insulina, se logro disminuir en un 71% el numero de pacientes con ese tipo de tratamiento y se modifico a hipoglucemiantes orales complementado con el la dieta y el ejercicio.
