Análisis comparativo de los criterios utilizados en el Servicio de Urgencias del Centro Médico ABC frente a los de Ottawa para la toma de radiografía de tobillo y pie

Abstract
Los criterios de Ottawa han sido utilizados en servicios de urgencias de todo el mundo para identificar clinicamente la posibilidad de fracturas en tobillo y mediopie y con ello determinar la necesidad de tomar radiografias. Estos criterios han demostrado tener 100% de sensibilidad y 100% de valor predictivo negativo. Su uso ha reducido el costo de la atencion medica y el tiempo de espera sin demeritar la calidad de la atencion. En este estudio se comparo la cantidad de radiografias de tobillo y pie solicitadas en el Servicio de Urgencias del Centro Medico ABC Campus Observatorio, con las indicadas segun los criterios de Ottawa. Fueron incluidos 54 pacientes. Se realizaron 49 radiografias y se encontraron nueve fracturas. De seguir los criterios de Ottawa, solo se habrian tomado radiografias a la mitad de los casos, entre los cuales hubiesen quedado incluidos los nueve con fracturas. La sensibilidad y el valor predictivo negativo fueron de 100% con los dos criterios. La especificidad y el valor predictivo positivo resultaron menores para el Hospital ABC (11 y 18%, respectivamente) frente a los criterios de Ottawa (60 y 33%) (p < 0.001). Si se hubieran aplicado los criterios de Ottawa, se habria ahorrado tiempo y dinero a 22 de los 54 pacientes estudiados, sin disminuir la calidad de la atencion medica
