IDegLira bei Patienten mit Typ 2 Diabetes: Wirksamkeit und Sicherheit nach 1 Jahr

Abstract
Hintergrund: In einer vorangegangen Studie über 26 Wochen zeigte IDegLira, eine Kombination von Insulin degludec (IDeg) und Liraglutid (Lira), Vorteile bezüglich der glykämischen Kontrolle und milderte die Nebenwirkungen der einzelnen Komponenten. Methoden: Patienten mit Typ 2 Diabetes setzten ihre zugeteilte Behandlung mit 1-mal-täglich IDegLira, IDeg oder Lira plus Metformin ± Pioglitazon in dieser 26-wöchigen Verlängerung fort. Ergebnisse: Von 1663 Erwachsenen in der Hauptstudie (mittleres Alter: 55 Jahre, BMI: 31,2 kg/m 2 ), nahmen 78,8% an der Verlängerungsstudie teil: IDegLira 665 (79,7%); IDeg 333 (80,4%); Lira 313 (75,4%). Der HbA 1c wurde gegenüber Baseline um 1,8% (IDegLira), 1,4% (IDeg) bzw. 1,2% (Lira) auf 6,4%, 6,9% bzw. 7,1% (geschätzter Behandlungsunterschied (ETD): -0,46 (IDegLira vs. IDeg) und -0,65 (IDegLira vs. Lira), beide p < 0,0001) reduziert; 78% der Patienten unter IDegLira erreichten einen HbA 1c < 7% vs. 63% für IDeg und 57% für Lira. Die mittlere NPG war für IDegLira (5,7 mmol/l) und IDeg (6,0 mmol/l) vergleichbar und für Lira mit 7,3 mmol/l höher (p < 0,0001 für IDegLira vs. Lira). Am Studienende lag die tägliche Insulindosis unter IDegLira 37% niedriger (39 E) als mit IDeg (62 E) (ETD: -23,4, p < 0,0001). IDegLira führte zu einer mittleren Gewichtsreduktion von 0,4 kg gegenüber einer Gewichtszunahme mit IDeg (ETD: -2,80, p < 0,0001). Die Hypoglykämierate war unter IDegLira 37% niedriger als unter IDeg (p < 0,0001). Mit IDegLira hatten weniger Patienten unerwünschte gastrointestinale Ereignisse als mit Lira (Übelkeit: 10,3% vs. 22,3%). Schlussfolgerung: Eine verbesserte Blutzuckereinstellung und ein günstigeres Sicherheitsprofil unter IDegLira in der Verlängerungsphase unterstützt die Nachhaltigkeit der Wirkungen von IDegLira über 1 Jahr.
