Diagnostik adulter Granulosazelltumoren unter Berücksichtigung aktueller Forschungsergebnisse

Abstract
Fragestellung: Die pathologische Diagnostik adulter Granulosazelltumoren des Ovars (aGCT) beinhaltet neben der histomorphologischen Begutachtung traditionell den Einsatz immunhistochemischer Keimstrangmarker. Der immunhistochemische Nachweis von Keimstrangmarkern kann bei der differenzialdiagnostischen Abgrenzung verschiedener Tumorentitäten sehr hilfreich sein, ist letztlich jedoch nicht typspezifisch. Im Gegensatz hierzu steht mit der kürzlich neu beschriebenen Mutation im Forkhead box protein L2 ( FOXL2 ) ein für aGCT spezifischer und sensitiver molekularer Test zur Verfügung. Ziel des aktuellen Projektes war die retrospektive Begutachtung eines großen aGCT Kollektives unter Berücksichtigung der FOXL2 Mutationsanalyse als zwischenzeitlich zusätzlich verfügbarer diagnostischer Methode. Methodik: 47 nach morphologischen und immunhistochemischen Kriterien in den Jahren 2000 – 2013 an der Universitätsfrauenklinik Tübingen als aGCT diagnostizierte Tumoren. FOXL2 Mutationsanalyse mittels TaqMan allelic discrimination assay. Abschließend erneute histopathologische Begutachtung durch drei Gynäkopathologen. Ergebnis: Die FOXL2 Mutationsanalyse konnte in sämtlichen Fällen erfolgreich durchgeführt werden, hierbei konnte die Mutation in 37 Fällen (79%) nachgewiesen werden. Die verbleibenden 10 Fälle wurden einer erneuten histopathologischen Beurteilung zugeführt. Dabei wurde in 4 Fällen die Diagnose eines aGCT trotz negativem Mutationsstatus bestätigt. 2 Fälle wurden als Sertoli-Leydig-Zell-Tumoren, 3 Fälle als Ovarialkarzinome und 1 Fall als Thekom reklassifiziert. Schlussfolgerung: FOXL2 Mutationsanalysen können die pathologische Diagnostik ovarieller Keimstrang-Stromatumoren sinnvoll ergänzen, insbesondere in Fällen, bei welchen differenzialdiagnostisch ein aGCT erwogen wird, eine verlässliche Typisierung jedoch mithilfe konventioneller Histologie, Histochemie sowie immunhistochemischer Färbung von Keimstrangmarkern nicht möglich ist. Dies könnte zukünftig besonders im Zusammenhang mit erhofften typspezifischen Therapiemodalitäten maligner Keimstrangtumoren Bedeutung erlangen.
