Abstract
われわれは虫垂炎手術を契機にAFP産生胃癌を発見し,その後Cronkhite-Canada症候群(CCS)と診断した1例を経験した.症例は68歳,男性.右下腹部痛を主訴に当科紹介受診となった.急性虫垂炎による汎発性腹膜炎の診断のもと,回盲部切除術を施行したが,触診にて胃に腫瘤を認め胃癌が疑われた.精査にて1型の胃癌と全大腸に数mm大の多数のポリープを認めた.爪甲萎縮・脱毛・色素沈着・蛋白漏出性腸炎を認めたためCCSと診断した.胃癌に対して幽門側胃切除術を施行した.径7cm大の1型の腫瘍が胃体部大彎に存在し,病理所見は低分化型腺癌であり一部にhepatoid様増生を認め, AFP染色にて陽性であった.胃,大腸の背景粘膜は腺窩上皮の過形成と嚢胞状拡張を認め, CCSに典型的な所見であった. CCSは癌合併率が高いが,検索しえた限りではAFP産生胃癌合併例は認められず,極めて稀な症例と考えられ報告する.
