Spongiöse hypertrophe Kardiomyopathie: eine neues Erscheinungsbild der hypertrophen Kardiomyopathie bei Patienten mit FHL1-Mutation

Abstract
Ziele: Die Mutation im Four-and-a-Half LIM Domain 1 Gen (FHL1) führt zu einer X-chromosomal vererbten Erkrankung mit Atrophie der posturalen Muskulatur und einer generalisierten Hypertrophie der Skelettmuskulatur. Hauptsächliche Todesursache der X-linked myopathy with postural muscle atrophy (XMPMA) ist ein respiratorisches und kardiales Versagen. Ziel dieser Studie war, mögliche kardiale Veränderungen bei Patienten mit FHL1-Mutation nachzuweisen. Methode: 16männliche Patienten mit dem Nachweis einer homozygoten Mutation im FHL1-Gen wurden prospektiv in die Studie eingeschlossen. Bei allen Patienten wurde ein EKG, eine Echokardiographie mit Strain Rate Bestimmung und ein Kardio-MRT nativ und nach Kontrastmittelgabe durchgeführt. Als Vergleichsgruppe wurden 22 gesunde männliche Probanden gleichen Alters untersucht. Ergebnis: 71% Prozent der Patienten zeigten EKG-Veränderungen wie T-Negativierungen. Echokardiographisch fand sich eine diastolische Funktionsstörung sowie eine eingeschränkte systolische Pumpfunktion (radial systolic strain: 24,6±11,8% vs. 43,2±14,8%). Bei allen 16 Patienten ergab die Funktionsauswertung der MRT eine erhöhte linksventrikuläre Masse (115,1±25,3g/m 2 vs. 89,0±15,6g/m 2 ) und eine schwammartige Morphologie des linksventrikulären Myokards. In den Spätaufnahmen nach Kontrastmittelgabe konnte bei 9 von 16 Patienten eine intramurales late enhancement als Ausdruck einer myokardialen Fibrose nachgewiesen werden. Schlussfolgerung: XMPMA-Patients zeigen durchweg elektrische, funktionale und morphologische kardiale Veränderungen. Die eingeschränkte systolische und diastolische Funktion geht einher mit linksventrikulärer Hypertrophie und einer myokardialen Fibrose als mögliche Ursache eines späteren Herzversagens.
