Beurteilung des Krankheitsverhaltens (Montreal/Vienna Klassifikation B-Wert) von M. Crohn durch die MR-Enterografie

Abstract
Einleitung: Die MRT des Dünndarms stellt die von den neuen Leitlinien neben dem Ultraschall empfohlene radiologische Modalität zur Beurteilung des Dünndarms bei M. Crohn (MC) dar. In unserer Studie vergleichen wir die Ergebnisse der MRT bezüglich der Operationsergebnisse mit Histologie zur Beurteilung des Krankheitsverhaltens (disease behavior, B-Klassifikation) der Vienna bzw. Montreal Klassifikation. Material und Methodik: 79 prospektiv eingeschlossene Patienten mit MC, die innerhalb von 4 Wochen nach einer MRT Untersuchung eine Darmresektion hatten, wurden evaluiert. Dabei wurde die MRT B-Klassifikation von 2 Radiologen im Konsensus durchgeführt. Als Goldstandard galt die von dem Chirurgen angegebene B-Klassifikation (B1 entzündlich, B2 strikturierend, B3 penetrierend) der Erkrankung. Basierend auf OP Situs und Histologie. Ergebnisse: Chirurgisch wurden 5% als B1, 21% B2 und 74% als B3 beurteilt. Radiologie und Chirurgie waren bei 97% aller Patienten (inter-observer agreement 0,937) identisch. Eine chirurgische b“ wurde radiologisch als B1 eingestuft, ein weiterer B3 radiologisch als B2. Strikturen hatten mit 96% die höchste radiologische Sensitivität, die Diagnose von Konglomerattumoren hatte mit 81% die niedrigste Sensitivität. Abszesse hatten den niedrigsten PPV mit 68% bei einer Spezifität von 88%. Die besten Korrelationen wurden mit 0,895 bei Fisteln detektiert. Konklusion: Mit der MRT lässt sich das B-Verhalten bei MC exzellent bestimmen. Dies bestätigt die MRT als herausragendes bildgebendes Verfahren zur Beurteilung von penetrierenden und strikturierenden Erkrankungen und somit als geeignetes Werkzeug vor geplanten chirurgischen Interventionen.
