Wound Infection Preventive Effects of the Lap-Protector in Open Acute Appendicitis Surgery: a Retrospective Study

はじめに : 急性虫垂炎手術における術後創感染は,入院期間の延長により患者のQOLを損ね,医療費に与える影響も大きい.当院では,術後創感染防止策として,2008年4月から開腹下急性虫垂炎手術症例全例に対し,ラッププロテクター(Lap-protector;以下,LPと略記)を使用している.本稿では,当院の症例を振り返り,開腹下急性虫垂炎手術におけるLPの創感染防止効果につき検討した.対象と方法 : 2006年10月から2009年10月までの開腹下急性虫垂炎手術症例99例を対象とした.これらを2008年3月までのLP導入前の49例,2008年4月以降のLP導入後の50例に分け,両群間で臨床項目につき比較検討した.結果 : 両群間に患者背景,術前白血球数,術前CTでの腹水の有無,虫垂の炎症の程度に有意差はなかった.LP使用群では,非使用群と比較して術前CRP値は有意に高く,術前CTで膿瘍合併例が多い傾向があるなど,より炎症の進行した症例が多かったが,創感染合併例は認めなかった.一方,LP非使用群では7例に術後創感染を認めた.創感染合併例は創感染非合併例に比べ,入院日数は平均で4.6倍,入院費の出来高比較は2.9倍で,双方とも差は有意であった.考察 : LPの使用は,開腹下急性虫垂炎手術後創感染を明らかに減少させた.医療経済,患者の術後QOLの点から,開腹下急性虫垂炎手術におけるLPの有用性が示された.
