MR Angiografie (MRA) der supraaortalen Gefäße bei 3,0 T: intraindividueller Vergleich Gd-basierter Kontrastmittel

Abstract
Ziele: Intraindividueller Vergleich eines 1.0 molaren (M) Gadolinium- (Gd) basierten Kontrastmittels (GBCA) und zwei 0.5 M GBCA bei equimolarer Dosierung zur dynamischen und statischen MRA. Methode: Bei insgesamt 20 Probanden (29 ± 6 Jahre) wurden jeweils 3 MRA Untersuchungen bei 3,0 T durchgeführt. Die drei evaluierten GBCA (Gadobutrol, Gadobenate dimeglumine, Gadoterate meglumine) wurden in randomisierter Reihenfolge mit mindestens 48h Intervall zwischen den einzelnen Untersuchungen appliziert. Vor und 45 Minuten nach den Untersuchungen wurden die Kreislaufparameter bestimmt. Die Gesamtdosis der verabreichten GBCA war auf jeweils 0,1mmol/kg Körpergewicht (KG) beschränkt. Es wurden 0,07 bzw. 0,03 mmol/kg KG für die statische und die dynamische MRA verwendet. Die qualitative Bewertung erfolgte durch zwei zueinander geblindete Radiologen. Zur quantitativen Bewertung wurden SNR und CNR berechnet sowie die Gefäßwandschärfe mittels einer semi-automatischen Software bewertet. Die statistische Auswertung wurde mittels der Cohen's Kappa Statistik, des Wilcoxon Rank Tests und „mixed effect“ Modellen durchgeführt. Ergebnis: Qualitativ wurde Gadobutrol bei exzellenter Übereinstimmung (k 0,66 – 0,83) der bewertenden Radiologen signifikant besser als Gadoterate meglumine (p = 0,0002) und gleichwertig zu Gadobenate dimeglumine (p = 0,057) bewertet. SNR und CNR waren mit Gadobutrol signifikant höher als bei beiden anderen GBCA (SNR p = 0,046/0,033, CNR p = 0,043/0,026). Die quantitative Bewertung der Gefäßwandschärfe zeigte keine signifikanten Unterschiede zwischen den verwendeten Kontrastmitteln. Schlussfolgerung: Bei equimolarer Dosierung zeigt 1,0 M Gadobutrol ein höheres SNR und CNR als Gadobenate dimeglumine and Gadoterate meglumine bei qualitativ ebenfalls höherer Bildqualität. Das konzentrationsbedingt geringere applizierte Volumen von Gadobutrol führt zu keiner signifikant gesteigerten Gefäßwandunschärfe.
