Abstract
ADL や QOL のみならず，生命予後にまで影響することが明 らかにされている 1) 。 ビスホスホネート(BP)は，破骨細胞に作用し，骨 吸収を阻害することから，骨粗鬆症による骨折の予防 のほか，癌の骨転移，悪性腫瘍に伴う高カルシウム血 症の治療などにも使用され，その有用性が示されてい る 2) 。そして，骨粗鬆症治療においては，BP 製剤は第 1 選択薬とされている 1) 。一方，2003 年に Marx 3) が BP 製剤により顎骨壊死が誘発されたことを報告し， その後，同様の報告が続出し，日本でも 2006 年に BP 製剤投与患者に対する侵襲的歯科治療への注意喚起が なされた。 現在，アメリカ歯科医師会やアメリカ口腔外科学会 のポジションペーパー 4,5) に加え，わが国においても BP 関連顎骨壊死(Bisphosphonate related osteonec-rosis of the jaw：BRONJ)の病態およびその対応につ いての指針 2,6) が見られるようになった。しかしなが ら，現状は，歯科治療における BRONJ の発症を予防 することが主眼となっており，歯周治療の予後に BP 製剤がどの様な影響を及ぼすかは不明である。本症例 報告レビューでは，BP 製剤投与骨粗鬆症患者に歯周 外科を伴う歯周治療を行い，結果的には良好な経過を 得ているものの，現在であれば，エビデンスに欠ける 点から異なる術式を選択していたとも考えられる症例 について考察する。
