Abstract

目的:多形腺腫は上皮性成分と問葉系成分からなる多彩な組織像を示す. その多彩性は主にさまざまな形態を示す腫瘍性筋上皮細胞に起因している. 今回われわれは, 多形腺腫における腫瘍性筋上皮細胞の細胞診上の特徴像を把握する目的で組織像と細胞像の対比を行った.対象:当院にて組織学的に診断され, 細胞診標本が得られた唾液腺多形腺腫症例30例 (耳下腺22例, 顎下腺8例) について比較検討を行った.成績:組織標本上, 腫瘍性筋上皮細胞の基本的な細胞形態は, 類上皮細胞型・淡明細胞型・類形質細胞型・紡錘形/星状細胞型の4種類の型に分類できた. そのほかに特殊な形態を示す腫瘍性筋上皮細胞として, 類軟骨細胞型・扁平上皮化生細胞型・大型奇怪細胞型・類基底細胞型・類脂肪細胞型などが認められた. これら各型の腫瘍性筋上皮細胞は, 細胞診標本においても確認できた. 特に組織像をよく反映し, 細胞診標本上認識しやすかった腫瘍性筋上皮細胞は, 類形質細胞型・紡錘形/星状細胞型であったが, 淡明細胞型および類脂肪細胞型を明らかな腫瘍性筋上皮細胞として同定することは困難であった.結論:組織像と比較対比することにより, 多形腺腫にみられる多彩な腫瘍性筋上皮細胞の細胞学的特徴を詳細に把握することができた. この多彩な腫瘍性筋上皮細胞の形態を把握することが, 多形腺腫の細胞診断には重要であると考える.
