Objectives: To determine the phenotypic variations in epileptic encephalopathies of infancy and childhhod that are responsive to pyridoxine (vitamin B6) administration: both acute and maintenance. The role of the EEG in helping diagnose these disorders is elucidated. Methods: A community-based series of 1500 children with epilepsies are studied by clinical examination, neuroimaging (CT and MRI), and follow-up for at least 2 years. All had EEGs with 10–20 electrodes, polygraphic recording of EOG, EKG, respirations and chin EMG. Pyridoxine HCl 50+50mg IV during EEG monitoring was followed by 100mg po daily, with clinical and EEG followup. Responders had complete seizure control, with or without AEDs. Non-responders had no change in EEG or seizure control. Results: Of 12 cases studied, 4 (33%) were responders: neonatal seizures, infantile spasms with hypsarrhythmia (West Syndrome), early childhood epilepsy with hyperphosphatasia (presented at AES 2005)and juvenile generalised epilepsy. Non-responders (8 or 66.7%) include neonatal HIE, symptomatic West Syndrome, Ohtahara Syndrome and MISF. Conclusion: The phenotypic variations in the epileptic encephalopathies of infancy and childhood range from neonatal seizures (pyridoxine-deficient and dependent) to infantile spasms, early and late childhood seizures, even to juvenile idiopathic epilepsy. The EEG may be diagnostic in certain cases. The genotypes still require definiton.
