Chronische Diarrhoe, Riesenfalten, Alopezie und Onycholyse – Erstdiagnose eines Cronkhite-Canada-Syndroms

Abstract
Kasuistik: Die Vorstellung der 68-jährigen Patientin erfolgte wegen seit sechs Monaten anhaltender breiiger Diarrhoen und eines Gewichtsverlusts von 15kg. Klinisch imponierten eine ausgeprägte Alopezie und onycholytische Nagelveränderungen. Laborchemisch fielen eine Anämie, sowie ein ausgeprägter Eiweiß- und Vitaminmangel auf. Gastroskopisch fand sich eine Verbreiterung und ödematöse Schwellung der Magenfalten mit multiplen kleinen Polypen. Endosonographisch zeigte sich hier eine diffuse Verbreiterung der Mukosa ohne Anhalt für wandüberschreitendes Wachstum. Koloskopisch und in der Kapselendoskopie imponierten im Bereich des Dünn- und Dickdarm ebenfalls multiple polypöse Veränderungen der Schleimhaut.In der histopathologischen Begutachtung der Magen- und Kolonbiopsien wurde das klinisch vermutete Cronkhite-Canada-Syndrom bestätigt.Aufgrund der ausgeprägten klinischen Symptomatik wurde eine Therapie mit Prednisolon und Mesalazin eingeleitet. Parallel erfolgte eine parenterale Substitution von Vitaminen und Mineralien.Unter dieser Therapie sistierten die Diarrhoen und es kam zu einer deutlichen klinischen Besserung.
