Erreichen des NPG-Ziels ohne nächtliche Hypoglykämien: eine gepoolte Auswertung von Studien mit Insulin degludec im Vergleich zu Insulin glargin bei Typ 2 Diabetes

Abstract
Fragestellung: Insulin degludec (IDeg) ist ein neues Basalinsulin mit langer und stabiler blutzuckersenkender Wirkung, welches nach s.c-Injektion Multihexamere bildet. Diese Eigenschaften können dazu führen, dass weniger nächtliche Hypoglykämien im Vergleich mit anderen Basalinsulinanaloga auftreten und mehr Patienten das Ziel für die Nüchternplasmaglucose (NPG) erreichen. Methodik: In dieser gepoolten Auswertung von 4 offenen, randomisierten Treat-to-Target- Studien, in denen die Patienten (n = 2380) entweder IDeg oder Insulin glargin (IGlar) einmal täglich zur festen Tageszeit in Kombination mit OADs über 26 oder 52 Wochen erhielten, wurde untersucht, wie viele Patienten mit T2D den NPG-Zielwert (< 90 mg/dl [5 mmol/l]) ohne nächtliche bestätigte Hypoglykämien erreichten. Bestätigte Hypoglykämien waren definiert als PG < 56 mg/dl (3,1 mmol/l) oder schwere Hypoglykämien, die Fremdhilfe erforderten, sowie nächtliche bestätigte Hypoglykämien, definiert als Hypoglykämien, die zwischen 00:01 und 05:59 Uhr auftraten. Der Endpunkt wurde mit einem logistischen Regressionsmodell untersucht, mit Behandlung, Geschlecht, Studie, antidiabetische Behandlung beim Screening und Herkunft als fixe Faktoren sowie Alter und Baseline-NPG als Kovariablen. Ergebnisse: Ein größerer Anteil von Patienten erreichte das NPG-Ziel (39,1% vs. 24,1%) und weniger Patienten erlebten nächtliche bestätigte Hypoglykämien (84,8% vs. 86,3%) mit IDeg versus IGlar. Die Wahrscheinlichkeit, das NPG-Ziel ohne bestätigte nächtliche Hypoglykämie zu erreichen, war mit IDeg 82% höher: geschätzte Odds Ratio IDeg/lGlar 1,82 [1,49; 2,22] 95% Kl. Schlussfolgerungen: T2D-Patienten erreichten das NPG-Ziel ohne bestätigte nächtliche Hypoglykämie mit IDeg mit höherer Wahrscheinlichkeit als mit IGlar. Diese Erkenntnisse können wichtige Implikationen für das Erreichen der Zielwerte der Blutzuckereinstellung in der klinischen Praxis haben.
