Rak piersi w IV stopniu zaawansowania jest zazwyczaj chorobą nieuleczalną.Do niedawna sądzono, że operacyjne leczenie ogniska pierwotnego nie przedłuża życia chorych w IV stopniu zaawansowania.Niniejszy artykuł jest oparty na przeglądzie publikacji dotyczących tego zagadnienia, które ukazały się pomiędzy styczniem 2000 r. a majem 2010 r. i zawierały dane na temat możliwego wpływu miejscowej operacji raka piersi w IV stopniu zaawansowania na wyniki leczenia.Niedawno opublikowane badania wskazują na znaczne korzyści w zakresie czasu przeżycia wynikające z miejscowego leczenia ope racyjnego chorych na raka piersi w stadium rozsiewu.W większości publikacji przedstawiano związek pomiędzy doszczętną operacją ogniska pierwotnego (szerokie wycięcie miejscowe lub mastektomia) a poprawą przeżywalności (mediana czasu przeżycia operowanych chorych była w wielu badaniach niemal dwukrotnie większa).Część badań nie wykazała jednoznacznej korzyści z leczenia operacyjnego lub korzyść tę przypisywała innym czynnikom (głównie przedoperacyjnej chemioterapii).Obecnie trwają kontrolowane badania z randomizacją ma jące wyjaśnić rolę miejscowego leczenia operacyjnego u chorych na raka piersi w IV stopniu zaawansowania klinicznego.
