Two Cases of Cronkhite-Canada Syndrome Associated with Colorectal Cancer

Abstract
症例1は65歳の男性. 主訴は食欲不振, 下痢, 血便. 内視鏡検査で胃と大腸にポリポーシスを認め, Cronkhite-Canada症候群 (CCS) と診断された. また, 上部直腸に高分化型腺癌を認めた. ステロイド治療で症状が改善した後, Hartmann式直腸切断術を施行した. 症例2は57歳の男性. 主訴は下痢, 味覚異常. 内視鏡検査で胃と大腸にポリポーシスを認めた. 直腸S状部の隆起性病変に対して内視鏡的粘膜切除術を施行したところ, 組織学的には高分化型腺癌であり, 周囲粘膜はCCSによる粘膜病変であった. CCSのポリープは非腫瘍性ポリープとされているが, 近年, 消化器癌の併存例が多数報告されている. 一般に予後は不良であるとされているが, 治療法の改善に伴い多くの長期生存例を認めている. このような中で, 当症例のように癌を併存する症例も増加することが考えられ, CCSに対しては注意深い経過観察が重要であると考えられた.
