To the Editor: Ours is a simple observational study (1), which was conceived to shed light on a question raised by our Fellow at the time of signout. None of us had previously published on, nor even much considered, the controversy behind the existence of actinic granuloma. Our null hypothesis was that actinic granuloma is granuloma annulare, and that the only difference between the two concepts is the presence of elastophagocytosis and loss of elastic tissue in the former. Our observation was that lesions with loss of elastic tissue (actinic granuloma) do, in fact, differ from lesions of granuloma annulare in a number of additional ways. The typical lesion of actinic granuloma shows sarcoidal granulomata confined to the level of elastosis in the superficial dermis, contains numerous multinucleate histiocytes, and lacks mucin; the typical lesion of granuloma annulare shows palisading granulomata with central mucin accumulation, has few multinucleate histiocytes, and bears no discernible spatial relationship to the elastotic material. We end the study with hypotheses that spring from these observations: that actinic granuloma represents histiocyte-mediated destruction of actinically-altered elastic tissue, and that granuloma annulare represents a histiocytic reaction to a focus of relatively indestructible altered collagen. There is indeed some degree of histologic overlap between actinic granuloma and granuloma annulare, as there is between innumerable inflammatory dermatoses; however, by appropriate statistical analysis of the differences along a variety of parameters, we have shown that each difference is unlikely to be a random finding. By evaluating our observations with a “cursory glance” and interpreting multiple statistically significant differences as “negligible”, Dr. Ackerman has chosen the path of obfuscation rather than elucidation. As the remainder of Dr. Ackerman's discussion does not refer directly to our findings, we will refrain from commenting on it in detail. Differences of opinion are bound to persist between those who pursue a pragmatic approach based on hypothesis testing by methodical observation, and those who pursue a philosophical approach based on metaphor, analogy and historical context. In closing, we choose not to persuade, but rather to invite readers to reexamine, with an inquisitive eye, the observations and arguments presented in our study and in Dr. Ackerman's commentary.
