Abstract
本邦では，一酸化炭素（carbon monoxide: CO）中毒による自殺が多いが，CO中毒の合併症として意識障害が主に注目され，心機能低下についてはあまり知られていない。患者は29歳の男性である。車内でコンロを焚いて倒れているところを発見され，当院へ救急搬送された。来院時の意識レベルはJapan coma scale 200，脈拍数145/分，血圧124/80mmHg，carbon monoxide hemoglobin（CO-Hb）49.6%であった。CO-Hb値からCO中毒を疑い，気管挿管下に100%酸素を投与した。心電図上で虚血性変化を認めなかったが，心エコー検査の左室駆出率（left ventricular ejection fraction: LVEF）低下とトロポニンIの異常値から，心筋障害を併発していると診断した。 高気圧酸素療法（hyperbaric oxygen therapy: HBOT）施行後，意識状態が改善すると同時に尿量の増加とLVEFの改善を認めた。以上より，CO中毒患者の診療においては，脳への障害だけでなく心筋障害も念頭に置き，心エコー検査とトロポニンI測定をルーチンに行う必要があると考えられた。またCO中毒に対するHBOTは意識障害だけでなく，心筋障害に対しても有効である可能性が示された。
