Abstract
Il était de comparer les résultats pressionnels à un an postopératoire avec le MicroShunt® d'InnFocus avec ou sans chirurgie de la cataracte associée, selon différents emplacements et concentrations de la MMC.Il s'agit d'une étude rétrospective sur deux centres avec deux chirurgiens (France et République dominicaine).Les adultes devant bénéficier d'une chirurgie filtrante pour glaucome chronique à angle ouvert ont bénéficié d'un MicroShunt® sur un œil. L'effet de la concentration et du lieu d'application de la MMC a été étudié sur la PIO et sur la diminution du nombre de collyres anti-glaucomateux à un an.Quatre-vingt-sept yeux ont été étudiés avec un suivi d'un an. Vingt-trois yeux traités avec de la MMC à une concentration de 0,4 mg/mL près du limbe ont eu une baisse de PIO de 55 %, passant de 23,8 ± 5,3 mmHg en préopératoire à 10,7 ± 2,8 mmHg à un an. Le nombre de molécules hypotonisantes par patient a été diminué de 85 %, passant de 2,4 ± 0,9 à 0,3 ± 0,8. Trente et un yeux traités avec de la MMC à une concentration de 0,2 mg/mL près du limbe ont eu une baisse de PIO de 52 %, passant de 27,9 ± 6,7 mmHg en préopératoire à 13,3 ± 3,3 mmHg à un an. Le nombre de molécules hypotonisantes par patient a été diminué de 88 %, passant de 2,5 ± 1,4 à 0,5 ± 1,0. Trente-trois yeux traités avec de la MMC à une concentration de 0,4 mg/mL loin du limbe ont eu une baisse de PIO de 38 %, passant de 25,4 ± 7,9 mmHg en préopératoire à 15,7 ± 4,6 mmHg à un an. Le nombre de molécules hypotonisantes par patient a été diminué de 72 %, passant de 2,9 ± 1,0 à 0,8 ± 1,3. Il n'y a pas eu de complications menaçant la fonction visuelle.Le MicroShunt® d'InnFocus est une chirurgie filtrante dont l'efficacité dépend de la localisation et de la concentration de la MMC utilisée.To compare intraocular pressure (IOP) at one year with the InnFocus MicroShunt® with or without cataract surgery with according to placement and concentration of mitomycin C (MMC)A retrospective two-center, two-surgeon study (France and Dominican Republic).Adults with POAG requiring filtering surgery. Thirty-one eyes treated with 0.2 mg/mL MMC close to the limbus demonstrated a 52% reduction in IOP from 27.9 ± 6.7 at baseline to 13.3 ± 3.3 mmHg at one year. Topical glaucoma medication/patient was reduced 88% from 2.5 ± 1.4 to 0.5 ± 1.0. Thirty-three eyes treated with 0.4 mg/mL MMC deep in the pocket demonstrated a 38% reduction in IOP from 25.4 ± 7.9 at baseline to 15.7 ± 4.6 mmHg at one year. Topical glaucoma medication/patient was reduced 72% from 2.9 ± 1.0 to 0.8 ± 1.3. There were no sight-threatening long-term adverse events.The InnFocus MicroShunt® is a filtering surgery whose efficacy is related to the location of application and concentration of MMC used.
