20 Thesen zu Gesundheit, Partizipation und Empowerment im Gespräch zwischen Arzt und Patient

Abstract
Aus den zusammengeführten Perspektiven der Gesundheitsförderung und der Systemtheorie lässt sich die Bedeutung der ärztlichen Gesprächsführung für die Gesundheit und Gesundung des Patienten neu argumentieren: Gesundheitsförderung als Prozess der Selbstbestimmung verstanden, stellt die Qualität der Gesprächsbeteiligung von Patienten in den Mittelpunkt salutogenetischer Prozesse. Damit wird die Arzt-Patient-Interaktion selbst zum zentralen Instrument der Wiederherstellung und Förderung von Gesundheit – zum „Herz der Medizin“, das in allen Gesprächsphasen Potenziale der Patientenselbstbestimmung und damit der Gesundheitsförderung eröffnet. Zunächst wird in vier Thesen die grundlegende systemtheoretische Sichtweise auf Gesundheit, Gesundheitsförderung, Empowerment und Partizipation entwickelt. Weitere fünf Thesen vertiefen die Bedeutung der Arzt-Patient-Interaktion für Gesundung und Patientenselbstbestimmung. Die folgenden neun Thesen bringen zentrale Ergebnisse der empirischen Forschung zur Arzt-Patient-Interaktion vor dem Hintergrund dieses Bedeutungszusammenhangs auf den Punkt. Hier werden Gesprächssteuerungsmöglichkeiten des Arztes in Hinblick auf Patientenselbstbestimmung analysiert und empirisch belegt. Diese Analyse baut auf einem ersten systematischen Syntheseversuch von gesprächsanalytischer Forschung zur Arzt-Patient-Interaktion auf. Der Artikel schließt mit zwei Thesen zu Forschungs- und Praxisperspektiven für dieses Kernstück der Medizin. Damit tritt uns die ärztliche Gesprächsführung als potenzielle Gesundheitsförderungsmaßnahme entgegen und kann im Sinne der „Reorientierung der Gesundheitsdienste“ in der Ottawa-Charta für Gesundheitsförderung als vielversprechendes Entwicklungsfeld der Gesundheitsförderung neu verstanden werden.
