【目的】遠位横止めステムを用いた人工股関節再置換術の術後成績を調査した.【対象】1997―2010年までに遠位横止めステムを用いた人工股関節再置換術症例18例19股を対象とした.【検討項目】原疾患,再置換原因,大腿骨骨欠損状態(Paprosky分類),骨折状況(Vancouver分類),使用ステム,調査時X線所見,合併症,術後JOAスコアを検討した.【結果】原疾患はOA14股,RA2股,ON1股,大腿骨頚部骨折2股,再置換原因は無菌性ゆるみ8股,感染6股,骨折3股,ステム破損2股であった.骨欠損はゆるみ・破損例でPaprosky type III B 7股,IV 9股,骨折例でVancouver分類typeB2 2股,B3 1股,使用ステムはHackstep 4股,Cannulok 8 股,デルタ‐LOCK 7股であった.調査時X線ではゆるみ5股,スクリュー折損2股を認めた.合併症は術後感染1股,感染再燃3股,脱臼5股,術中骨折1股であった.調査時JOAscoreは平均59点であった.【考察】術前骨欠損の大きな症例や感染症例ではステムのゆるみを認める症例が多かった.
