The intent of this critical assessment was to review key areas of the clinical and technical work flow in a critical care environment as it pertains to the variations that exist between 2 cartridge-based blood gas systems, namely, the Siemens RAPIDPoint 405 and the IL GEM4000. Cartridge-based blood gas systems on the surface would seem to completely remove the need for maintenance or technical/ clinical intervention; however, careful clinical assessment highlights a number of areas that require consideration. On replacement of cartridges, considerable variations exist between manufacturers before the system is fully operational. Maintenance of quality and reliability of results is manufacturer dependant and adds time to either cartridge installation or daily procedures. Sampling time between manufacturers and time for next analysis vary greatly. Within this assessment, areas of work flow that require careful consideration clinically and technical within the institution and their effect on work flow patterns when consolidating technology to a cartridge-based blood gas system were highlighted.
