Extremely early CT signs in hyperacute ischemic stroke

Abstract
脳梗塞急性期のt-PAの静脈内投与は,「発症3時間以内,CTで早期虚血変化がないかまたは軽微を十分に満たす場合」を必要条件としている.しかし実際には,頭部単純CTでのレンズ核の不明瞭化・島皮質や他の皮質の皮髄境界不明瞭化・脳溝の不明瞭化などの早期虚血変化は微細な変化に過ぎない.我々は,発症6時間以内の内頸動脈系の初発脳梗塞連続98例を対象に,Alberta Stroke Program Early CT Score(ASPECTS)に基づき患側半球を10領域に分け,早期虚血変化を判定した.超早期,特に発症90分以内のCTでレンズ核と島皮質の両方に早期虚血変化を認める例は,保存的治療にてもその後に血腫形成型の出血性変化が高率にみられた.今後はこれらの例での血栓溶解療法の適応や抗血栓療法の開始時期について,再検討する必要があると思われる.また一部の例でMRI拡散強調画像と対比し,CTで早期虚血変化を認めた部位は,全例が梗塞に陥るわけではなく一部は可逆的であることを明らかにした.
