目的: 子宮頸部細胞診の結果判定と報告様式について検診受診者, 細胞診判定者 (細胞検査士, 細胞診専門医) ならびに細胞診専門医以外の産婦人科医における理解と対応の相違点を検討し, 現行のクラス分類の問題点を The Bethesda System (TBS) と対比することを目的とした.方法: 子宮頸がん検診受診者 182 名に細胞診判定結果の理解についてのアンケート調査を行った. 細胞検査士 62 名, 細胞診専門医 41 名, 細胞診専門医以外の産婦人科医 30 名に判定基準についての理解とその対応について電話や文書によるアンケート調査を行った.成績: 受診者の約 83%はクラス分類が理解しにくいと回答し, わかりやすい説明を要望していた. 推定病変とクラス分類との相関について, 扁平上皮系の軽度と高度異形成では 95%以上の高い一致率であったが, 中等度異形成では 48%がIII a, 45%がIII b, 7%が III と判定していた. また同じ組織診を推定しても該当するクラス判定に診断者間でのずれが目立っていた.結果判定後の対応についてはクラスIII a の低精査率の背景が示された.結論: 多岐にわたる細胞像について従来のクラス分類に当てはめることの限界が示された. TBS では扁平上皮, 腺上皮, それ以外の細胞について区分され, 診断者間により整合性のある判定が期待でき, 標本の適切性を判定の条件とすることや細胞採取方法を銘記することなど精度管理上, また医療安全の立場からも評価できる. しかし, ASCUS, ASC-H などの新しい用語, 本邦でルーチンには使用されていない HPV 検査を視野に入れた区分, HSIL に中等度異形成が包括されていることなど, 検討すべき課題が残されている.
