Die Glow-5 Studie: Wirksamkeit und Sicherheit von Glycopyrronium verglichen mit verblindetem Tiotropium (jeweils einmal täglich) bei Patienten mit COPD

Abstract
Einleitung: Da Glycopyrronium (GLY; NVA237) und „open-label“ Tiotropium (TIO) im Rahmen einer klinischen Studie bei COPD eine ähnliche Wirksamkeit zeigten [1], wurde GLOW5 konzipiert, um die Wirksamkeit und Sicherheit von GLY mit TIO in einer verblindeten Studie zu vergleichen. Methode: In dieser multizentrischen, 12-wöchigen verblindeten Studie wurden Patienten (Pats) mit moderater bis schwerer COPD auf einmal täglich GLY 50 µg oder TIO 18 µg 1:1 randomisiert. Primärer Endpunkt: Demonstration der Nicht-Unterlegenheit (NU) zwischen GLY und TIO für trough FEV 1 bei Wo 12 (NU-Spanne: -50 ml). Weitere Ergebnisse: FEV 1 -AUC 5 min-4h an Tag 1, Transition-Dyspnoe-Index (TDI), St George's Respiratory Questionnaire (SGRQ), Gebrauch von Notfallmedikation, Sicherheit und Verträglichkeit. Ergebnisse: Von 657 randomisierten Pats (GLY: n = 327; TIO: n = 330; Durchschnittsalter: 63,5 Jahre, durchschnittlicher post-bronchodilatatorischer FEV 1 : 53,5%) schlossen 95,9% die Studie ab. In Wo 12 zeigte GLY eine Nicht-Unterlegenheit zu TIO für trough FEV 1 (LSM = 1,41 l für beide Gruppen, 95% CI: -0,032, 0,031 l). GLY zeigte eine signifikant bessere FEV 1 -AUC 5 min-4h an Tag 1 gegenüber TIO (p < 0,001). In Wo 12 zeigten GLY und TIO verglichen mit den jeweiligen Ausgangswerten einen vergleichbaren Effekt auf TDI- und SGRQ-Gesamtpunktzahl und den Gebrauch von Notfallmedikation. Die Gesamtzahl der unerwünschten Ereignisse war in den GLY-(40,4%) und TIO-(40,6%) Gruppen vergleichbar. Schlussfolgerung: GLY zeigte eine effektive Bronchodilatation und war verblindetem TIO nicht unterlegen und bezüglich der morgendlichen Bronchodilatation an Tag 1 gemessen im 4-Stunden Intervall nach Erstgabe der jeweiligen Medikation sogar überlegen. GLY und TIO zeigten ähnliche Verbesserungen bei Dyspnoe, dem Gesundheitszustand, dem Notfallmedikationsgebrauch und waren jeweils sicher und gut verträglich. Referenz: [1] Kerwin, E. et al.
