A Case of Tuberculous Peritonitis Combined with Gastric Cancer.

Abstract
症例は83歳男性で, 25歳時に肺結核の既往があり, 9カ月前に上行結腸癌に対し右半結腸切除を受けていた. MU領域後壁の3型の胃癌に対してTS-1, CDDPによるNeoadjuvant chemotherapy後に開腹手術を施行した.開腹時,腹膜および腸問膜に粟粒大の無数の小結節と回腸の部分的狭窄を認め,腹膜播種を疑ったが,腹腔洗浄細胞診および小結節の術中迅速病理検査では悪性所見は認められなかった.胃全摘および回腸結腸吻合部切除を行った.病理組織検査にて小結節は,抗酸菌染色陽性の菌体成分が認められ,結核性腹膜炎の合併と診断され,術後抗結核剤による治療を開始した.近年,結核は再興性感染症のひとつとして注目されている.結核性腹膜炎は稀であるが,担癌症例の開腹時に播種性病変がみられた場合,念頭に置く必要がある.また手術侵襲や化学療法による結核再燃の可能性に留意すべきと思われる.
