Characteristics of memory impairment in elderly patients with cerehral infarction.

Abstract
Objective To investigate the characteristics of memory impairment in elderly patients with cerebral infarction.Methods 76 older patients with cerebral infarction and 76 age, sex and education-matched healthy volunteers were enrolled prospectively and consecutively.Based on brain infarction locations shown in MRI, the patients were classified into subgroups, including frontal group, temporal group, parietal and occipital group, basal ganglia group, brain stem group and cerebellar group.Based on the infarction size, the patients were classified into big infarction group, middle infarction group, small infarction group, lacunar infarction group and mix infarction group.Montreal Cognitive Assessment (MoCA) was used to evaluate cognitive function, Clinical Memory Scale (CMS) , Fuld Object Memory Evaluation (FOM) and Rapid Verbal Retrieve (RVR) were applied to evaluate memory function in both the patients and the controls.Results Of all the 76 patients, 44 (57.9%) had low MoCA score (26).Compared with control group, the scores in MoCA and every memory function evaluation were declined in the study group and in its subgroups, including frontal group, temporal group, parietal and occipital group, basal ganglia group (P0.01).Furthermore, the scores in MQ, FOM and RVR were declined in the frontal group, temporal group, parietal and occipital group compared to those in the basal ganglia group ( P 0.05); whereas, no significant difference was observed in every memory function evaluation among the frontal group, temporal group, parietal and occipital group (P0.05).In the frontal group and temporal group, the score in every memory function evaluation was lower in the middle infarction group than that in the small infarction group and lacunar infarction group (P0.01) , and the score was lower in small infarction group than that in the lacunar infarction group (P0.01).In the parietal and occipital group, the basal ganglia group, the score in every memory function evaluation was lower in the middle infarction group than that in the small infarction group and lacunar infarction group (P0.01).No difference in every memory function evaluation was present between small infarction group and lacunar infarction group ( P 0.05). Conclusions The cognitive impairment incidence in elderly patients with cerebral infarction was high.The characteristics and degree of memory impairments are associated with the infarction location and size.
