Edmonton Symptom Assessment Scale (ESAS) er et symptomkartleggingsskjema som benyttes i lindrende kreftbehandling. I den senere tid har forskning vist at pasienter har problemer med å forstå terminologien i skjemaet og at det skjer misforståelser ved bruk. Hensikten med denne kvalitative studien var å undersøke hvordan et utvalg palliative pasienter fortolket og fylte ut spørsmålene i ESAS, samt å undersøke hvordan de opplevde bruken av skjemaet. Til dette ble det benyttet kognitivt intervju, som er en velegnet metode for å identifisere og korrigere problemer ved spørsmål i spørreskjemaer. Under slike intervjuer bes informanter om å tenke høyt rundt alle detaljer i forbindelse med utfylling av skjema. Således kan man få innblikk i prosesser som personen selv ikke er oppmerksom på. Dette kan føre til at potensielle feilkilder og misforståelser oppdages. Resultatene fra intervjuene i denne studien viste at det forekom feilkilder, både i forbindelse med utfylling og bruk av skjemaet. Spesielt symptomer som smerte, angst/uro og trist/deprimert ble tillagt forskjellig meningsinnhold av informantene. Skalaene, som skal indikere intensitet av symptomer, ble også fortolket forskjellig. Dersom dette ikke tas hensyn til, vil det forringe verdien av skjemaet, både som behandlingsunderlag og kvalitetsmål. Modifikasjoner av skjemaet, forankring av skjemaets relevans hos pasienten og rutinemessig oppfølging av svarene, er nødvendig for at ESAS skal ha den nytteverdien som det er tiltenkt. Slike endringer vil også kunne bidra til bedret kommunikasjon og økt fokus på psykososiale problemer.
