A glicemia e a quantidade de glicose no sangue e a verificacao de seus parâmetros torna-se importante na prevencao e deteccao de hipo e hiperglicemia. A hiperglicemia induzida por estresse ocorre com frequencia em pacientes criticos internados em unidades de terapia intensiva e tem sido associada ao aumento de morbimortalidade em diabeticos e nao diabeticos. A proposta da insulinoterapia e melhorar as condicoes do paciente, propor maior qualidade e seguranca no servico prestado. O objetivo deste estudo e desenvolver um protocolo de insulinoterapia para a unidade de emergencia medica – Retaguarda UNICAMP. Utilizou-se publicacoes relacionadas a protocolos de insulinoterapia, em bases de dados indexadas eletronicas (Lilacs, Bireme, Scielo) sites, manuais e protocolos institucionais. Um protocolo eficiente agiliza a assistencia, diminui a chance de erros de calculo, diminui o tempo da assistencia e proporciona um servico seguro e eficaz. Optou-se por uma adaptacao do Protocolo do Hospital de Ottawa, onde sua seguranca e efetividade foram comprovadas por ser de facil compreensao, nao contem calculos complexos, direciona seu controle a equipe de enfermagem e requer pouca intervencao medica. Como objetivo terapeutico, os niveis glicemicos foram de 140 a 180 mg/dl proposto pelo Estudo Multicentrico Normoglycemia in Intensive Care Evaluation-Survival Using Glucose Algorithm Regulation (NICE- SUGAR), por ser atual e seguro. A selecao do protocolo requer uma investigacao cuidadosa, e a escolha deve levar em consideracao o tipo de paciente e as experiencias e habilidades da equipe de enfermagem, que tambem contribuem para o sucesso dos protocolos.
