Abstract
I was touched by the dozens of responses that I have received to this editorial,[1] sharing both thoughts on the issue itself as well as very moving personal experiences. I agree completely with Mr. Galva[2] that the conditions that have created the structure of our current arrangements have dramatically changed; today's job market, family structures, and patterns of disease all argue strongly for breaking the old links between employment (and nuclear families) and health insurance coverage. As Rev. Brown[2] and many others have pointed out, the Canadian single-payer system is one clear, working example of how to break this link, although as reflected in Dr. Cahn's comments,[2] there is reason to believe that this would be harder to achieve in America given our history and particular culture (particularly our distrust of government). Another interesting model is the one we use in primary and secondary education; we make the provision of basic and comprehensive services a public responsibility (eg, through tax-funded public schools that are free and available to all), but allow many private options to exist for those who choose to use them (eg, tutoring programs, charter schools, and private schools). Although this sort of solution raises obvious equity concerns, it would certainly be better than the travesty of the nonsystem that we have today.
