Abstract
BACKGROUND: Cerebellar mutism (CM) is a condition characterized by a significant lack of speech or loss of speech and other symptoms following posterior fossa (PF) surgery. Its biological cause remains unknown. METHOD: We conducted a retrospective cohort-comparison study among patients diagnosed with childhood brain tumours from 2004 to 2015 from McMaster University Medical Centre. Data was extracted from the Pediatric Brain Tumour Study Group database, with 147 cases total. The 11 patients who acquired CM post-surgery were matched by age and tumour type with 11 patients who did not acquire this condition after PF brain tumour removal. MRI and surgical parameters were collected. RESULTS: Preliminary results show that 91% of patients with CM had tumour adherent to the floor of the fourth ventricle, compared to non-cases in whom 45% had tumour invading the fourth ventricle. MRI reviews demonstrated that 36% of non-CM patients had infarcts or bleeds in comparison to 73% of patients who had acquired CM post surgery. 36% of patients with CM had a definite residual tumour following the surgery in which they acquired CM, whereas 64% of non-CM patients had a residual. CONCLUSION: There is a trend to suspect that patients with tumours adherent to 4th ventricle floor are at an increased risk to develop CM. Vascular involvement and aggressive resections may also be risk factors. These findings should be implemented into the surgical planning of posterior fossa tumour resection.
