Abstract
Objective: To determine concordance of PS variants in non-consanguineous married couples. Background: The etiology of idiopathic Parkinson’s disease (IPD) and most other Parkinson syndromes (PS) is unknown. Definite diagnosis is based on histological examination of brain. Observations on Lewy body (LB) pathology extension into transplanted fetal tissue suggests transmissibility of disease. There are rare clinical reports of conjugal PS cases but no autopsy confirmed series has been reported to our knowledge, to assess possible transmission by spousal relationship. Methods: Movement Disorders Clinic Saskatchewan (MDCS) has been conducted since 1968. Autopsy is offered at no cost to those attending the MDCS. So far, 537 cases including 392 PS cases seen at MDCS have come to autopsy. We identified couples where both male and female partners were seen in our clinic, had clinical diagnosis of PS, and each came to autopsy. We excluded drug-induced parkinsonism, spinocerebellar ataxia, and those who had a history of essential tremor preceding the onset of Parkinson’s disease. Results: Five couples where both the wife and husband had PS and each came to autopsy were identified. A) In two couples, each spouse had Lewy body (LB) Parkinson’s disease but in one, one partner had strong family history of pathology verified LB disease. B) Both members of one couple had Progressive Supranuclear Palsy but one had Lrrk2 mutation. C) One spouse had LB disease and one had Multiple System Atrophy. D) There was usual tauopathy in one and LB disease in the other spouse. Conclusions: Our data indicate that neither of the common degenerative variants of PS is transmitted sexually or by close personal contact.
