Background: The ongoing enthusiasm for ultrasound (US)-guided lower extremity (LE) nerve blocks has spawned a multiplicity of divergent methods. Accordingly, a literature search of level 1 evidence (randomized trials) was undertaken to determine the optimal approaches and techniques for US-guided lower extremity nerve blocks. Objectives: To review the evidence from randomized  controlled trials (RCTs) pertaining to approaches and techniques for US-guided LE nerve blocks. Methods: The MEDLINE, EMBASE, and PUBMED databases were searched by using the keywords: lumbar plexus, psoas compartment, psoas sheath, fascia iliaca, three-in-one, 3-in-1, femoral nerve, obturator nerve, saphenous nerve, adductor canal, lateral femoral cutaneous nerve, lumbosacral plexus, sacral plexus, sciatic nerve, common peroneal nerve, tibial nerve, popliteal, popliteal sciatic. These keywords were then combined with block, nerve block, ultrasound, and ultrasound-guided. The search was limited to RCTs involving human subjects and published in the English language. Results: The search criteria yielded 8 RCTs and 14 RCTs pertaining to nerves of the lumbar plexus and sacral plexus, respectively. The average sample size was 63 subjects. Most trials provided sample size justification, blinded assessment and Jadad scores 3.Based on the available evidence, all methods for US-guided femoral catheter insertion result in similar efficacy However, the short axis technique requires a shorter performance time. Compared to its nerve-targeting counterpart, the subinguinal technique for lateral femoral cutaneous nerve block provides a markedly higher success rate. All approaches for saphenous nerve block yield comparable efficacy However, proximal methods may be associated with a higher incidence of vastus medialis muscle weakness.  Anterior and posterior subgluteal sciatic nerve blocks provide similar success rates but only the anterior approach can be carried out with the patient in a supine position. Circumferential deposition of local anesthetic should be preferred to a punctual injection for both the subgluteal and popliteal sciatic nerve. Compared to a popliteal sciatic block proximal to the neural bifurcation, targeted tibial and peroneal nerve blocks provide a higher success rate. Although recent evidence suggests that subparaneural injection may constitute the best technique for US-guided popliteal block, further trials are required to assess its safety. Conclusion: Published reports of RCTs provide evidence to formulate limited recommendations regarding optimal approaches and techniques for US-guided LE nerve blocks.
