Flow-Pressure Relationship in Fontan-Palliated Patients

Abstract
Simultaneous pressure and Doppler flow patterns were recorded in the inferior vena cava (IVC) and main pulmonary artery (PA) of 6 subjects in whom nonvalved atriopulmonary anastomosis had been performed. All patients were asymptomatic without any pressure gradient at the level of the anastomosis. In the PA, forward flow was always biphasic showing a first peak in atrial systole and a second one between the end of the R wave and the end of the P wave. A backward flow of short duration was observed right after the R wave. Velocity and PA pressure curves had similar patterns: maximal pressure values corresponding to the highest velocities. In the IVC, forward flow was always biphasic with the highest peak during ventricular systole and a smaller one between the end of the T wave and the end of the P wave. In contrast to PA, the highest forward velocities in IVC were associated with the lowest intravascular pressure. Inspiration enhanced all forward flows.
