A Case Report of Cronkhite-Canada Syndrome Successfully Remitted by Surgical Treatment

症例は50歳代の女性で, 2002年12月中旬より腹痛, 食欲低下, 味覚異常が出現し当院内科を受診. 手指皮膚色素沈着, 爪甲萎縮, 舌炎, 2か月で約10kgの体重減少を認めた. 精査の結果, Cronkhite-Canada症候群と診断, 保存的治療で寛解した. 2004年11月より再燃を認め, 保存的治療を行うも効果なく, 腸閉塞を続発した. 下部消化管検査で回腸から上行結腸にかけて広範な多発性顆粒状隆起と, 回腸末端部の閉塞を認め, 腸閉塞治療目的に腹腔鏡補助下回盲部切除術を施行した. 切除標本では回腸約25cmに及ぶ炎症性ポリープと高度浮腫を認めた. 術後経過は良好, 2POMには血中アルブミン値は4.4g/dlと著明に改善し体重は増加. 下痢, 色素沈着・脱毛・爪甲萎縮はいずれも寛解した. 保存的治療抵抗性Cronkhite-Canada症候群に対する外科治療は, 選択肢の一つになる可能性が示唆された.
