Abstract
Les enjeux éthiques soulevés par la désinstitutionnalisation des services offerts aux personnes atteintes de troubles mentaux sont abordés dans cet article sous l’angle de la justice sociale définie en termes d’accès égal à des soins adéquats en santé mentale et donnés en temps opportun. Après avoir défini brièvement l’origine et l’apport des institutions psychiatriques dans le domaine du soin en santé mentale, le phénomène de la désinstitutionnalisation de ces services au Québec est analysé relativement aux conséquences individuelles et sociales qu’une telle politique gouvernementale a entraînées. Dans cette optique, les difficultés, voire l’échec, dans la mise en place d’un suivi adéquat dans la communauté, le fardeau des familles, la situation précaire de certaines personnes à leur sortie de l’institution psychiatrique, sont mis en évidence. Sont décrits ensuite une philosophie et un modèle d’intervention dans la communauté, tels qu’expérimentés par la psychiatrie de secteur en France. Enfin, ces analyses permettent de préciser des conditions préalables à la mise en place dans la communauté d’un service de santé mentale adéquat et accessible à toute personne qui nécessite un tel service.
