OBJECTIVE: To examine the pattern and severity of WMH in a cohort of HIV-positive patients on antiretroviral therapy (ART) suffering from neurocognitive impairment. BACKGROUND: HIV-associated neurocognitive disorder (HAND) is characterized by impairments of working memory and executive functioning. This has been observed in up to 52[percnt] of patients from North American adult HIV clinics. Patients with HAND also demonstrate various degrees of WMH. In this study, we examined the correlation between cognitive impairment and HIV using standardized WMH visual rating scales. DESIGN/METHODS: The cohort is comprised of HIV positive adults of age 19 years or older who consented to provide data through the HIV Neurocognitive Disorder Clinic (NDC) at St. Paul’s Hospital, Vancouver BC, Canada. Data collected included demographics, medical and HIV-related history, neuroimaging (MRI), and cognitive screening tests with the HIV Dementia Scale (HDS) and Montreal Cognitive Assessment (MOCA). RESULTS: A total of 82 participants are currently enrolled in the study: 76 male and 6 female. The median age is 53 years (IQR 49-61). The median CD4 count was 535 cells/mm3 (IQR 400 cells/mm3 - 702.5 cells/mm3). Plasma viral loads range from 35 copies/mL to 157427 copies/mL. The median MOCA score was 23 (IQR 21-27) and the median HDS score was 13.3 (IQR 9-17). The burden of periventricular and deep WMH on T2-FLAIR MRI was evaluated in 67 participants using two already established visual rating scales (AWRMC and Fazekas). The median AWRMC score was 0.250 (IQR 0.00-0.500), and the median Fazekas score was 0.625 (IQR 0.500-1.00). The AWRMC basal ganglia scores significantly correlate with MOCA scores (R=0.288 ; p=0.0241). CONCLUSIONS: The findings suggest an association between the burden of WMH and neurocognitive function. The correlation between the severity of WMH and HIV-related ART and disease progression will be evaluated with further analysis of data.
