Synovial Fluids: Crystals and Culture: A One-Year Retrospective

Abstract
The objective was to document the number, type, stability, and laboratory findings of synovial fluids submitted to our hospital laboratories in a 12-month period (April 1, 2011–March 31, 2012). Our laboratories serve 1,000 hospital beds and 400,000 outpatients. All microscopy was performed by ourselves, and patient records were reviewed to determine the joint and crystals and bacterial culture results. Microscopy was repeated on 114 negative samples at 8 hours and about 50 positive samples up to 24 hours later. Of 255 orders, 70% were from knee; 10% from hips; 5% each from shoulders, elbows, and ankles; and 2% from wrists. Crystals were identified in 40%: 48% urate and 48% pyrophosphate; less than 1% both; 4% other crystal types (mostly unidentified and thought to be due to medications). The ratio of urate/pyprophosphate crystals was equal in knees; about 2:1 in elbows, ankles, and wrists; and 0:1 in hips or shoulders. None of 114 initially crystal-negative samples were positive on reexamination after 8 hours. No positive cases became negative. Only hip, knee, shoulder, and elbow fluids yielded positive bacterial cultures, 20% of which also contained pyrophosphate crystals. Organisms isolated were MRSA (19/31) Staphylococcus warneri (3), various streptococci (5), and miscellaneous (4). Knee fluids comprised the vast majority of fluids submitted with hips a distant second; 40% of knees yielded crystals; more from ankles and shoulders and least from elbows and hips. Urate and pyrophosphate crystals occurred equally in knees, but urate crystals were not found in hips or shoulders, and less than 1% of fluids contained both urate and pyrophosphate crystals. Of the fluids, 12% had positive cultures; 20% also contained crystals; all pyrophosphate. The majority of positive cultures were due to gram-positive cocci, mainly Staphylococcus aureus. Crystal findings appear to be very stable for 8 hours or more after sampling.
