Eine randomisierte Phase III Studie einer Kombinationstherapie Carboplatin/Paclitaxel mit oder ohne Bexarotene (Targretin®) bei nicht vortherapierten Patienten mit fortgeschrittenem oder metastasiertem Nicht-kleinzelligem Bronchialkarzinom (NSCLC)

Abstract
Hintergrund: Bexarotene (Targretin®) ist ein selektiver Retinoid X Rezeptor (RXR) Agonist. Nachdem in präklinischen Untersuchungen eine antitumoröse Aktivität gezeigt werden konnte, wurden im Rahmen einer Phase II Studie ermutigende Ergebnisse durch Kombination von Carboplatin/Paclitaxel mit Bexarotene mit einer medianen Überlebenszeit von 13,7 Monaten beobachtet. Im Rahmen einer randomisierte Phase III Studie erfolgte nun die weitere Prüfung dieser Substanz. Methode: 612 chemonaive NSCLC-Patienten im Stadium IIIb/IV und ECOG 0–1 wurden in zwei Gruppen mit Carboplatin/Paclitaxel plus Bexarotene (400mg/m 2 /Tag) versus Carboplatin/Paclitaxel randomisiert. In Erwartung erhöhter Triglyzeridwerte unter der Therapie, wurde parallel eine lipidsenkende Therapie durchgeführt. Die Patienten wurden vor Randomisierung nach Krankheitsstadium und Geschlecht stratifiziert. Primärer Studienendpunkt war die generelle Überlebenszeit, sekundärer Endpunkt die 2-Jahres Überlebenszeit. Ergebnisse: Weder im progressionsfreien Überleben (medianes PFS: 4,9 v 4,1 M, p: 0,061; Kontrollarm v Bexarotene) noch im Gesamtüberleben (medianes Überleben: 9,2 v 8,5 M; 2-Jahres ÜL: 16,3% v 12,4%, p: 0,2; Kontrollarm v Bexarotene) zeigte sich ein signifikanter Unterschied zwischen den Armen. Eine Verbesserung des Überlebens zeigte sich für Patienten mit stark erhöhten Triglyzeriden unter Bexarotene im Vergleich mit Patienten mit niedrigen Triglyzeriden (medianes Überleben: 12,4 M v 6,6 M; hohe Triglyzeride v niedrige Triglyzeride)
