Abstract
Introduction and Aims: Higher serum phosphate is associated with cardiovascular disease in the non-CKD population. Parathyroid hormone (PTH) and fibroblast growth factor-23 (FGF-23) regulate urinary phosphate re-absorption and become elevated in early CKD before serum phosphate is abnormal. We determined whether the absolute and relative acute change in PTH and FGF-23 in response to an oral phosphate stimulus was associated with the appearance of phosphate in the urine at 120 and 180 minutes. Methods: GFR was measured by inulin clearance. Urine phosphate was assessed by the ratio of urine phosphate to urine creatinine (Uphos:Ucr). 15 subjects consumed 1 cup of water that contained 3.9 mls (500 mg) of Phoslax ( monobasic sodium phosphate 2.4 g and dibasic sodium phosphate 0.9g/5 ml). PTH, FGF-23, phosphate and calcium were measured at 0, 15, 30, 45, 60, and 120 minutes. Uphos:Ucr was assessed at 0, 120 and 180 minutes. Students paired t-test was used to test for statistical differences in the post-test response over baseline. Spearman coefficients were used to examine the correlations between GFR, PTH and FGF-23 and Uphos:Ucr.
