Einleitung: Aktuelle GOLD Richtlinien empfehlen eine LABA/ICS und/oder LAMA Behandlung in COPD Patienten mit hohem Exazerbationsrisiko. FLAME ist die erste Studie, die den Effekt von IND/GLY (LABA/LAMA) gegen SFC (LABA/ICS) in Patienten mit erhöhtem Exazerbationsrisiko vergleicht. Im Folgenden werden Exazerbations- und Lungenfunktionsdaten mit und ohne vorherige ICS Therapie gezeigt. Methoden: FLAME ist eine 52-wöchige, multizentrische, doppelblinde Studie. Patienten mit post-bronchodilatorischem FEV 1 von 25% – 60% des Sollwerts wurden auf IND/GLY 110/50 µg o.d. oder SFC 50/500 µg b.i.d. randomisiert. Ergebnisse: 1893 (56,3%) der insgesamt 3362 randomisierten Patienten erhielten vor Studieneinschluss eine ICS-basierte Therapie. IND/GLY reduzierte signifikant die Rate „aller“ sowie „moderater oder schwerer“ Exazerbationen und verbesserte die Lungenfunktion im Vergleich zu SFC in Patienten sowohl mit als auch ohne vorherige ICS Behandlung (siehe Tabelle).
