Abstract
多発性骨髄腫(MM)に対する同種移植は1980年代に開始された。初期は減量のない全身放射線照射(TBI)に大量～超大量の細胞障害性薬剤を加えた骨髄壊滅的前処置で体内MM細胞の殺細胞効果ともに移植片の生着を目指した。Bologna大学の1名は移植後30年以上経過し、Karolinska大学の2名も25年以上経過しても再発の兆候がなく、本方法で治癒がもたらされた可能性が考えられている。しかし、多くの症例ではこの前処置での移植関連死亡の比率が高いことから、非骨髄壊滅的前処置(RIC)への移行が進んだ。最近の前処置は、まず自家移植で体内のMMの総腫瘍量を減らした後でRICでの同種移植を実施することが一般化し、非再発死亡率(NRM)は10～16%に低下した。6つの自家移植群と自家移植+RIC同種移植を比較したprospective RCTのうち、2つではPFSとOSの有意な延長効果が報告された。ただし自家移植ではNRMが低率であることと、新規薬剤の導入が相次いでいることから、同種移植を躊躇する傾向が認められる。このため自家移植後の病勢増悪期に実施されることが多い。これら新規薬剤は、同種移植の領域では移植片対宿主病(GVHD)の軽減や維持療法で有効性が検討されている。妥当な同種移植のあり方は、他の治療で再発した症例で行うこと、およびdel 17p、del 8p、gain 1qなどの難反応とされる染色体異常があり、他の治療では奏効率が低く、予後不良と判断される症例においては、upfrontでの実施も考慮することである、と考えられる。2016年1月時点では、同種移植はclinical trialの範疇で実施されることが望まれる。なおMMに対してはmesenchymal stem cellを用いたGVHD軽減、NK細胞を用いた抗MM作用強化、DLI、chimeric antigen receptor T-cells(CART-cells)やNK-cells、などの各種細胞療法も試みられるに至っている。
