Abstract
A 6-year-old girl, who was being followed for autoimmune thrombocytopenia, underwent a bone marrow evaluation as part of a routine work-up of thrombocytopenia persisting for more than six months. Physical examination was normal. Peripheral blood count showed: leucocyte count 8·9 × 109/l, haemoglobin concentration 120 g/l, platelet count 46 × 109/l and reticulocyte count 4·7 × 109/l (normal range 22–92 × 109/l). A peripheral blood film showed isolated thrombocytopenia and rare giant platelets. The bone marrow aspirate showed normal cellularity, with mildly increased numbers of unremarkable megakaryocytes. The most significant finding was the almost complete absence of erythroid progenitors. Pronormoblasts were present and had large nuclear inclusions and dissolving nuclear membranes (images, ×40 and × 100 objectives). Conventional cytogenetic analysis showed a normal female karyotype. Based on the bone marrow findings, the patient was evaluated for recent parvovirus B19 infection. On further questioning, a consistent history was elicited with the classic signs of “slapped cheeks” and a diffuse rash. Parvovirus IgM serology was positive, indicating a recent infection. The child had no clinical signs or symptoms of anaemia. Parvovirus B19 is a common childhood infection, with most children manifesting only the cutaneous findings. However, in an individual with shortened red cell lifespan, infection can lead to significant anaemia. The incidental finding of severe erythroid hypoplasia on this bone marrow examination illustrates that a host may be completely asymptomatic despite a significant effect on intramedullary erythropoiesis.
