Abstract

5例の外傷性小脳内出血を経験したので報告する.受傷機転は3例が交通外傷, 1例が不明, 1例は転落によるものであった.4例が後頭部に強い打撃を受けていたが, 1例は上顎に軽い打撃を受けた例であった.後頭部打撲の4例は後頭蓋窩減圧開頭血腫除去術が必要であった.4例とも後頭骨に骨折線がみられ, 3例で骨折下の硬膜に断裂がみられた.1例は術後に脳腫脹を起こし死亡したが, 3例は予後良好であった.上顎部打撲による症例は出血が少なく保存的に加療したが, MRIにて右前頭葉にshearingによると思われる病変を合併しており, 回転加速度を生じるような外傷機転による小血管の剪断が小脳に発生し, 出血性病変に変化した例ではないかと考えられた.
