Le cancer du sein est le plus fréquent des cancers chez la femme, 42 000 nouveaux cas sont découverts par an. Le diagnostic de métastases ganglionnaires est réalisé par la technique du ganglion sentinelle qui consiste à repérer et prélever le premier relais ganglionnaire axillaire potentiellement métastatique. La localisation de ce ganglion se fait grâce à l'injection d'un produit radioactif la veille de l'intervention puis d'un colorant bleu en peropératoire. Cependant, il persiste des inconvénients inhérents à cette procédure comme les risques de choc anaphylactique au bleu et l'exposition à la radioactivité. C'est pourquoi de nouveaux marqueurs ont été proposés : les Quantum Dots (QDs). Ce sont des nanocristaux de semi-conducteurs fluorescents extrêmement brillants, photostables et dont la longueur d'onde d'émission dépend de la taille. L'utilisation des QDs permet de localiser le ganglion sentinelle sans les inconvénients de la technique actuelle et grâce à une seule procédure peropératoire.L'objectif de ce travail était de déterminer le lymphotropisme et la biodistribution des QDs in vivo. Résultats :Après injection de 20 pmol en sous-cutané dans la patte d'une souris nude, les QDs sont observés dans le ganglion sentinelle à tous les temps testés (5, 15, 30, 60min et 24h) à la fois grâce à un spectrofluorimètre fibré, par microscopie de fluorescence et par extraction chimique. Le maximum est observé à 60 min et correspond à 2,4 % de la dose injectée. L'étude de biodistribution ne montre pas d'accumulation dans d'autres organes et le dosage plasmatique atteste l'absence de QDs de la circulation sanguine. L'étude de l'urine et des fécès pendant 48 h montre que les QDs ne sont pas excrétés.Conclusions et perspectives :Les Quantum Dots permettent de détecter efficacement et sélectivement le ganglion sentinelle, et ce même avec une très faible quantité (20 pmol). La quantité maximale a été détectée 60 min après injection et correspond à 2,4 % de la dose injectée. L'utilisation de Quantum Dots sans métaux lourds pourrait éliminer l'éventuelle toxicité et l'utilisation du proche infra-rouge est nécessaire pour une application clinique.
