Abstract
Small midline intracranial cysts in the region of the cavum veli interpositi have been observed with ultrasound in our fetal imaging institution with increasing frequency over recent years. The objective is to provide a description of the sonographic characteristics, frequency, and postnatal outcome in a cohort with isolated midline cysts. Ultrasound databases were queried with the keywords “midline intracranial cyst” between January 2012 and January 2016. Birth outcome data were reviewed. Postnatal imaging and clinical follow-up were provided. In a 4-year period, 68 cases of isolated midline intracranial cysts were identified. The 6 cases identified as the cavum vergae were excluded. The remaining 62 cases, cysts were located in the region of the quadrigeminal plate cistern. Given the potential differential diagnosis of arachnoid cyst, pineal cyst, and true cyst of the cavum veli interpositi, all cases were followed with serial prenatal ultrasound examinations. All cysts (with one exception) measured less than 10 mm in diameter at diagnosis, and none enlarged on prenatal follow-up sonography. Fifteen cysts disappeared on ensuing scans. In one patient, the cyst initially measured 10 mm and enlarged slightly on subsequent scans. The newborn was considered to be normal on postnatal follow-up. Postnatal outcomes excluded pregnancies with unrelated adverse perinatal complications. A single fetus underwent postmortem ultrasound and the midline cyst was found to represent a posterior continuous extension of the third ventricle. All remaining cases had normal neonatal and/or postnatal clinical records. In this cohort, midline cysts in the region of the qradrigeminal plate cistern were associated with a benign outcome. This may provide reassurance to both patients and caregivers and may be of value in prenatal counselling.
