Ziel der Studie: Die Fallbeschreibung eines Kindes mit Therapie-refraktärer systemischer Vaskulitis und die Dokumentation des Therapieansprechens auf TNF-Blockade. Methode: Studiendesign: Prospektive Fallbeobachtung eines 12-jährigen Patienten mit Polyarteritis nodosa (PAN). Studienintervall: Diagnose im Alter von 13 Monaten, Beoachtungs- und Erkrankungsdauer: 11 Jahre. Datensammlung: prospektive, standardisierte Befunderhebung und Dokumentation von a) klinischen Daten inklusive Erkrankungsaktivität, b) Funktionsstatus und Lebensqualität (CHAQ), c) Labordaten inklusive Entzündungsmarkern und d) Behandlungsprotokollen inklusive Wirkung und Nebenwirkungen der Therapien. Ergebnisse: Der Fall eines 12-jährigen, karibischen Jungen mit 11-jähriger Erkrankungsdauer einer systemischen PAN Vaskulitis wird berichtet. Dystrophie: Im Alter von 12 Jahren ist er 114cm groß (unter 3. Perzentile) und wiegt 22kg (unter 3. Perzentile). Organbeteiligung: Leber (Fibrose plus Budd Chiari), Nieren (Nierenarterienstenose, Bluthochdruck), Blut (Anämie, Thrombozytose), Gelenke (Polyarthritis, Myositis) und Haut (stark schmerzhafte kutane Knoten). Therapien: Kortison (Puls und oral), Methotrexat (MTX) (oral und sc), Azathioprin und Cyclophosphamid (3 Blocke i.v. dann oral). Die Kortisonbehandlung wurde aufgrund anhaltend aktiver Erkrankung zu keiner Zeit unterbrochen. Er benötigte mindestens 1mg/kg Prednison pro Tag zur Kontrolle der Hautbeteiligung. Infliximab wurde 11/2001 begonnen, Startdosis war 5mg/kg i.v. alle 4 Wochen. Subjektiv verbesserte sich sein Befinden sehr rasch, die Hautläsionen heilten erstmalig ab. Die Kortisondosis konnte langsam reduziert werden. Nach 6 Monaten Infliximab-Behandlung (5mg/kg/Dosis) plus Prednison plus MTX entwickelte er jedoch neue, ulzerative Hautläsionen. Infektionen konnten ausgeschlossen werden und Infliximab wurde in erhöhter Dosis (10mg/kg) erneut gestartet. Nach 14 Monaten Infliximab-Behandlung ist sein Allgemeinbefinden dramatisch gebessert, der CHAQ Disability Index ist 0.87 (11/02: 1.3), alle Hautläsionen sind verheilt, die Anämie ist nicht mehr nachweisbar und alle Entzündungsmarker sind deutlich gebessert (CRP 71 jetzt 10.6mg/dl). Zusammenfassung: TNF-Blockade mit Infliximab stellt eine sichere und wirksame Behandlungsmöglichkeit der Therapie-refraktären systemischen PAN Vaskulitis im Kindesalter dar. Es kann im Rahmen der Behandlung zu Rezidiven kommen, die eine Dosisanpassung erforderlich machen.
