DIAGNÓSTICO DE TUBERCULOSIS LATENTE POR LA DETECCIÓN DE IFN-GAMMA, EN FAMILIARES DE TRABAJADORES AGRÍCOLAS DE PUEBLA MÉXICO, QUE MIGRAN A CANADÁ

Abstract
Introduccion: La tuberculosis, sigue siendo un reto de salud mundial. La poca captacion de casos sospechosos en grupos migrantes, colectar esputos adecuados para realizar la coloracion de Ziehl Neelsen y la demora en la interpretacion de los cultivos son parte del problema y favorecen el estigma del migrante vector . La deteccion de tuberculosis latente, por el metodo: IFN-gamma-TB, presenta ventajas en especificidad-sensibilidad, respecto a pruebas clasicas como IDR y baciloscopia. Correlacionar la clinica y los resultados de laboratorio inmunologico, podria evidenciar la dicotomia existente entre inmunidad humoral y celular en sujetos con igual evolucion de la infeccion tuberculosa. Material y Metodos : se encuesto y selecciono a 46 personas familiares de migrantes a Canada, con sospecha de TB, se les solicito muestras de esputo para realizarles baciloscopia, por tecnica de Ziehl Neelsen, adicionalmente se les efectuo flebotomia para la cuantificacion de interferon gamma, por la tecnica QuantiFERON-TB-Gold. Resultados : Solo 18 de los 46 sujetos seleccionados entregaron muestras de esputo, eliminandose 5, por deficiente calidad. Las 13 muestras seriadas procesadas resultaron negativas (100%) a BAAR por la coloracion de Ziehl Neelsen. En todos los sujetos se logro obtener muestras de sangre, para la deteccion y cuantificacion de IFN-gamma, de los cuales 6 (13%), resultaron con niveles de IFN-gamma, que sugieren TB latente. Conclusiones : Aunque la muestra estudiada es pequena, se confirma la dificultad para colectar muestras de esputo representativas para baciloscopia, en sujetos sospechosos de tuberculosis, sobre todo en mujeres. La determinacion de IFN-gamma, representaria una alternativa practica, para el diagnostico de tuberculosis latente, en grupos vulnerables de paises en desarrollo.
